• Must Register on our website to qualify.

• All form fields must be filled out completely and accurately.

• You cannot nominate yourself.

• The Perk team will randomly select one winner on the 5th of every month for the previous month.

• All entries will remain in the “pool” so you don’t need to nominate a person each month.

• A person can be nominated multiple times but can only win once.

• There are no refunds, exchanges, or credits for any items in.

• The items in the gift box are subject to change for each giveaway.

• All nominees must live in the US and have a valid shipping address, no PO boxes.

• Once a winner is randomly selected and contacted, they will have 10 days to confirm.

• If a winner is not confirmed in 10 days, a new name will be drawn.
